You may email your completed packet and eligibility documentation to forms@mercyhealthcenter.net or you may bring your packet to the office (700 Oglethorpe Ave. C7 Athens GA 30606).

M-Th 8:30AM - 4:30PM

Fridays 8:30AM - 3:00PM.

Eligibility Requirements:

  • Over 18 years of age

  • Lives in Clarke, Jackson, Banks, Barrow, Madison, Oglethorpe, Oconee, Morgan, Greene, Walton, Elbert, Hart, and Franklin counties.

  • Be uninsured

  • Income under 200-% of Federal Poverty Line
